Request a quote for our fee structure for on-site and after-hour testing.  We will provide a generic protocol to give you an idea of how the on-site testing would work.  We can edit the protocol to your company’s Policies and Procedures, we need the information below:

• Do you require a 5 panel or 10 panel drug test.
• Rapid test or Lab based test (all non-negative rapids are sent to lab for verification).
• We perform BAT (breath alcohol test) all on site after hours testing.
• In the event of a shy bladder (employees cannot provide urine sample) we will pivot to an oral fluid test and have it lab tested (please verify with your company’s policies and procedures for this option).
• Name, Phone Number, and email address for recipient of paperwork and results
• Name, Phone Number, and email address for recipient of invoices.
