  • Driver applications
  • 10-year employment history for CDL drivers
  • Previous employer histories (3 years)
  • Previous employer drug and alcohol checks (2 years)
  • Current medical cards and certificates
  • Copy of CDL or road tests for non-CDL drivers
  • Annual driving record from state agency (3 years)
  • Annual review of driving record
  • Annual certification of violations, completed by the driver

Alcohol and controlled substances testing documentation is another major component of DOT mock audit preparation. This includes pre-employment, random program, and post-accident test results.

Reasonable suspicion training for supervisors should be documented, and drivers must have a minimum of six follow-up tests in the first 12 months following a positive test result.
